Welcome to the Oral and Maxillofacial practice of Drs. Nader Moavenian, D. Cameron Braasch, Jonathan D. Bean and Imran Ahson. Thank you for selecting our practice for your comprehensive Oral and Maxillofacial health care needs. Enclosed is information that is intended to make your visit to our office both pleasant and efficient.
We kindly ask both new and established patients to complete the patient registration form and health questionnaire which contain key information.
Please complete these forms as soon as possible and either mail them back to our office, or bring them with you for your upcoming office appointment.
If you are unable to complete these forms prior to your office visit, please plan to arrive 15-20 minutes before your scheduled appointment time so that we may process your information in a timely fashion.
Please be sure to bring your medical and dental insurance cards with you to your appointment. Also, bring any x-rays, referral cards, or treatment requests that your dentist or physician may have given you when you were referred to our office.
Also, please review the acknowledgement of patient responsibility which outlines our financial policy.

Background Information, OMS
Oral and maxillofacial surgery (OMS) requires additional years of hospital-based surgical and anesthesia training after graduation from dental school. As oral and maxillofacial surgeons, Dr. Moavenian, Dr. Braasch, Dr. Bean and Dr. Ahson manage a wide variety of problems relating to the mouth, teeth and facial regions. They both practice a full scope of oral and maxillofacial surgery with expertise ranging from dental implant surgery and wisdom tooth removal to corrective jaw surgery. This also includes techniques designed to rebuild bone structure with minimal surgical intervention and optimal patient comfort. We can also diagnose and treat facial pain, facial injuries, and fractures.
Our staff is trained in assisting with Intravenous (IV) sedation or outpatient general anesthesia in our state-of-the-art office setting. Patients are continuously monitored during and after surgery.
Oral Surgery Staff
The surgical staff at PDOMS- Moavenian, Braasch, & Bean Oral & Maxillofacial Surgery are experienced, certified oral and maxillofacial surgical assistants, who assist in the administration of IV sedation and surgery. All staff are informed administrative personnel, well-versed in health and insurance policies, and are CPR certified.
